Support Coordination
Clearer plans. Better-connected supports. Less fragmentation
Support Coordination helps participants and families understand and use their NDIS plan, connect with suitable providers and keep the different parts of support working together. It can be useful for straightforward coordination needs as well as more complex plans involving multiple services, allied health professionals or significant changes in support.

This may be a good fit if
- the participant or family wants help understanding and using the NDIS plan
- suitable providers need to be identified and connected
- supports need better coordination or clearer roles
- a plan review, change or transition needs preparation
- the participant has a complex plan, multiple providers or allied health involvement
- support arrangements are changing and the next steps need to be organised clearly
What we focus on
Practical coordination, service connection, clearer communication, preparation for plan changes and keeping the participant’s priorities visible as different supports come together.
